The first time we ever saw a true Starfleet uniform undergarment in the modern era of Star Trek was in the Deep Space Nine season 2 episode "Paradise," during which the Sisko wore a gray tank top for a couple scenes (see right).
It seemed to be a basic tank top, nothing remarkable except perhaps its pointed neckline (Robert Blackman seemed to love his pointed/mitered necklines!)
For several more years, that was all we ever saw of the standard Starfleet-issue uniform underlayers.
